Community Case Management services help to increase your understanding of your illness and help you improve your quality of life by:

  • helping you follow your doctor’s treatment plan
  • reducing complications & reducing symptoms
  • helping you to answer your medical questions
  • removing barriers to service
  • working with you to develop your life plan to reach your lifestyle goals including returning to work.

To qualify for this free hospital service:

• You need to live in Waldo County

-or-

• You need to be under the care of a WCGH physician for COPD or CHF
• You can not be currently receiving skilled home healthcare or other skilled nursing facility services

We accept referrals from all sources, including physicians, discharge nurses, family members and self referral. Acceptance into the program is based on current case load.

Participation in this outreach program occurs by telephone, face to face, and with materials sent to your home. The amount of contact you have with the nurse is determined by your healthcare needs and how much contact you want.
